NOTICE REGARDING FORM 1095-C AVAILABILITY

In accordance with IRS Notice 2025-15 and the Paperwork Burden Reduction Act (PBRA) of 2024, we are notifying individuals of their right to request a copy of Form 1095-C for the 2024 calendar year. This notice applies to individuals covered by our health insurance plan as well as full-time employees.

How to Request Your Form 1095-C:

You may request a copy of your Form 1095-C by reaching out to us at any time between now and October 15, 2025. Please use one of the following methods to make your request:
